General Affidavit Format: Free Word Download with Verification

By Advocate Narinder Kumar, NK Associates · Updated October 2026

An affidavit is a written statement of facts that you swear or affirm to be true before a Notary Public or Oath Commissioner. Offices, banks, schools and courts ask for one for many purposes: proof of address, change of name, loss of documents, income, or that two names belong to the same person. A false affidavit is a criminal offence, so state only facts you know to be true.

Free to download and edit in MS Word, Google Docs or WPS Office on your phone.

How to fill and use it

Step 1: Use stamp paperPrint it on non-judicial or e-stamp paper of the value the receiving office asks for. Some offices accept plain paper; check first.
Step 2: Write one fact per paragraphReplace the bracketed lines with your facts, one fact in each numbered paragraph, in simple words. Add the purpose in the last paragraph.
Step 3: Do not sign in advanceSign as the deponent in front of the Notary Public or Oath Commissioner, carrying your original ID proof.
Step 4: Get it attestedThe Notary or Oath Commissioner attests your signature and puts a stamp and register number on it.

Common mistakes to avoid
