Name Change Affidavit Format and Gazette Process: Free Word Download

By Advocate Narinder Kumar, NK Associates · Updated October 2026

To change your name, or correct its spelling, you usually need three things: an affidavit declaring the change, an advertisement in newspapers, and for many purposes (passport, government jobs, certificates) a notification in the Gazette. Women changing their surname after marriage often need only the affidavit and marriage certificate, but check what the office concerned asks for.

Free to download and edit in MS Word, Google Docs or WPS Office on your phone.

How to fill and use it

Step 1: Make the affidavitPrint it on stamp paper of the value the office asks for, fill your old and new names exactly, and sign it before a Notary Public or Oath Commissioner.
Step 2: Advertise in newspapersPublish a short notice of the change in two newspapers, usually one English and one in the local language, circulating in your area. Keep the original newspaper pages.
Step 3: Gazette notificationApply for publication in the Gazette of India (Department of Publication) or the Punjab Government Gazette, as required by the office where you will use it, with the affidavit and newspaper cuttings.
Step 4: Update your recordsUse the affidavit, newspaper cuttings and Gazette copy to update Aadhaar, PAN, passport, bank accounts and certificates.

Common mistakes to avoid
